First of all, you need to back up your database. This is always a good thing to do before any upgrade because things can mess up and ruin your blog. A backup of the database is needed. There are plugins to help with this if you aren't sure what you are doing. If the upgrade does fail, you can create your blog again and try again.

You should disable all your plugins. Some argue that this is no longer required, but you might as well make the upgrade as smooth as possible. Disable these so that there won't be any issues that come up.

Check to make sure your plugins will work on the newest version of WordPress. If you need these plugins, you might consider to not upgrade until the plugin authors have everything ready for the newest version.

Login to your WordPress administration area and you will see an upgrade notification at the top of your screen. You can hit this and watch everything upgrade the way it's supposed to. It will let you know when it's done right.
